What is new or different about the Safety First manual Learning from Incidents?

The guide takes the position that you cannot learn from all incidents. The aim of Learning from incidents is to improve the safety system within which the incident took place. An incident may reveal a weak spot in a system, and investigation of the incident will increase understanding of the safety system. But investigation may also reveal that an incident is unique and that there is no need to adjust the safety system. Incident investigation is about discovering the flaws in the safety system and taking action when necessary.
