Project: Improvement Group Learning from Incidents (Pilot).
Description
The regional safety networks Masterclass Veiligheid Midden (MVM) and Zuid-West (MCV-ZW) will each start an Incident Learning Improvement Group within their network in 2021.
Within these Improvement Groups, a number of companies that are members of the MVM or MCV-ZW will set to work on implementing the step-by-step plan from the Guide to better learning from incidents in the (petro)chemical industry.
This improvement is based on five principles:
- Prioritise the investigation of the most risky incidents.
- Minor incidents offer the greatest learning potential.
- Carry out incident investigations according to the new view (accident causation model), which is based on the system approach.
- Organise a learning culture.
- The outcome of an incident investigation may also be that nothing needs to be done.
The improvement groups are facilitated by the MVM and MCV-ZW. The participating companies will be substantively supported in the implementation of the step-by-step plan by an SDN knowledge broker.

Purpose
Better Learning from Incidents at Organisational Level. This involves improving the safety system of a company within which the incident took place.
Expected result
Participating companies strengthen their capacity to learn from incidents at the organisational level by:
Information about the principles in the guide.
Training of employees in the application of the step-by-step plan. Training material will be made available for this purpose (complementary to e-learning via the SDN Academy).
An independent evaluation of the existing corporate approach to learning from incidents and a gap assessment versus the roadmap. A checklist will be made available for this purpose.
Implementation of an improvement plan in conjunction with the business priorities.
Sharing experiences and better practices with peers in the improvement group(s).

Why participate?
The pilot project 'Better learning from incidents' is an accessible way to improve the knowledge and skills within a company to comply with the Seveso Directive 2012/18/EU Annex iii under vi.
It offers the possibility of learning from each other more quickly as an Improvement Group to improve internal processes. In addition, it provides a platform for joint consultation with supervisors on the approach to learning from incidents.
Expected contribution per company: at least 40 hours of in-kind time of people working every day on safety for:
- Participation in training
- Carrying out the assessment
- Drawing up an improvement plan
- Participation in the RVN improvement group
