RIVM Report 2017-0085: Analysis of incidents at large companies with hazardous substances 2016-2017

Summary

The RIVM analyses annually incidents involving hazardous substances at large chemical companies. The analysis of 2016-2017 includes thirteen incidents. Three times a fire occurred and eight times dangerous gases or vapours were spread to the surroundings. In two cases, the impact was limited to the immediate surroundings of the incident. Nine of the thirteen incidents were the result of excessive pressure in an installation or incorrect human handling. Three people were injured, probably of a repairable nature.

Causes

Companies must carry out production processes and related work safely. The thirteen incidents involved various shortcomings in this respect. The measures that should have prevented these shortcomings were missing, were no longer functional or were not applied properly. Subsequently, the shortcomings were not detected in time because a means of control was lacking or was not properly applied.

In eleven incidents, underlying work procedures were not in order or not properly implemented. Other organisational requirements such as hazard alertness, competence of personnel and suitability of equipment went wrong less often.

Reason

This annual report is part of a long-term assignment of the Ministry of Social Affairs and Employment (SZW) to analyse incidents investigated by the Major Hazard Control Directorate (MHC) of the Inspectorate SZW. The MHC focuses on the safety of personnel at large chemical companies. The findings of this analysis can be used for inspection and enforcement strategies. Companies can use the insights to improve their safety policy.
