Measuring safety performance and learning from incidents

Measuring is knowing, and learning is the basis for improving safety.
Managers report safety figures within their organisation that are relevant, consistent, transparent, accurate and complete. Safety performance is expressed through 'leading' and 'lagging' indicators.
Managers shall ensure that procedures are in place for the internal reporting of all safety incidents, including 'near miss' incidents, and the external reporting of these in accordance with laws and regulations.
Investigation of an incident is carried out as quickly as practically possible by those responsible for the activities in which the incident occurred. The actual or possible consequences of the incident determine the depth of that investigation. Learning from incidents takes place through the sharing of information about the causes of the incident and the implementation of improvement actions to prevent recurrence.
