IOGP Report 456 - Process safety - recommended practice on key performance indicators

This report provides guidance on identifying leading and lagging process safety Key Performance Indicators (KPIs) for the upstream oil and gas industry. It is intended to enable companies to establish effective leading and lagging indicators that assess the ‘health’ of barriers that manage the risk of process safety events, particularly those that could result in a major incident. This second edition of Report 456, published in November 2018, incorporates revisions from the second edition of API RP 754. API published the second edition of API RP 754 in April 2016 and revised the Tier 1 and 2 material threshold categories.
