OvV report - Explosion of a natural gas condensate tank at the Nederlandse Aardoliemaatschappij, Warffum, 31 May 2005

On Tuesday 31 May 2005, a storage tank containing natural gas condensate from the Nederlandse Aardolie Maatschappij (NAM) in Warffum exploded during renovation work. Two employees of the contractor GTI were killed. A third GTI employee was seriously injured.

Lessons learned: - the granting of permission to carry out work on the basis of a work permit that indicated an incorrect risk level, namely "low", whereas the risk qualification should have been "high". - risks that arise directly from the process, process risks, which can also affect occupational safety and the recognition of these risks.
